－44－Ⅴ肝・その他〔鹿臨外会誌27巻〕３．18鮒田式胃壁固定具を応用した腹腔鏡下腹壁瘢痕ヘルニア修復術生駒外科医院生駒明、生駒茂、生駒達雄和文抄録腹壁瘢痕ヘルニアに対する腹腔鏡下手術は、メッシュをヘルニア門にあてがい、腹壁に固定する術式である。巨大な腹壁瘢痕ヘルニアではメッシュを固定するのに多数のタッカーを使用するので医療材料費が高額となる問題がある。メッシュの固定に、経皮的内視鏡的胃瘻造設術用の鮒田式胃壁固定具（胃壁固定具）を応用した。メッシュの全周を３cmおきにタッカーで固定した後、その間は胃壁固定具を用いてナイロン糸で皮膚からメッシュまで縫合固定した。しかし、胃壁固定具で縫合すると糸が皮膚の２ヶ所から出るので、結紮部が皮下に来るようにするために、片方の糸を縫合針に取り付けて持針器で把持し、把持した糸が出ている針孔から糸を皮下に入れ、他方の糸が出ている針孔から出して結紮する工夫を行った。この方法によって、メッシュの広い範囲の縫合固定が簡便に短時間に可能となり有用であった。またタッカーの使用個数も節約でき経済的にも有意義であった。索引用語：腹壁瘢痕ヘルニア、腹腔鏡下手術、メッシュ固定法英文抄録Laparoscopic Surgery for AbdominalIncisional Hernia Using Funada-styleGastropexy InstrumentAkira Ikoma, Shigeru Ikoma, Tatsuo IkomaIkoma Surgery ClinicIn laparoscopic incisional hernia repair,tackers and sutures are used to fix a meshto the abdominal wall. Here, we present anew simple procedure to enable the threadto be tied in the subcutaneous layer to fix themesh to the abdominal wall after the wholelayer suture using Funada-style gastropexyinstrument.Key Words: abdominal incisional hernia,laparoscopic surgery, meshはじめに腹壁瘢痕ヘルニアは、開腹手術の10～20%に発生するとされる1)2)。3.2%の症例が嵌頓や絞扼を起こして緊急手術を要するとされる3)。また筋委縮が進んで膨隆が巨大して、ますます治療が困難となる場合もある4)。積極的に手術すべき疾患である。手術方法として、従来の開腹手術に比して、低侵襲で、術後合併症、再発が少なく、入院期間が短い腹腔鏡下手術が普及しつつある5)6)7)8)。しかし巨大な腹壁瘢痕ヘルニアでは巨大なメッシュを固定するのに多数のタッカーが必要となり、医療材料費が高額となることが問題である。今回、経皮的内視鏡的胃瘻造設術用に開－44－
